LifePath’s Community Dining program offers hot, nutritious meals at locations across the Capital Region. Participants enjoy social activities, educational sessions and opportunities to connect with neighbors, helping reduce isolation and stay engaged in the community.
About the Program
Nationally, one in five adults over 65 is readmitted to the hospital within 30 days of discharge, and one in three within 90 days. LifePath created a grant-funded program, Providing Assistance and Support to Seniors in Transition, designed to reduce these rates and support recovery. Services may include:
Two home-delivered meals a day for up to two weeks, freshly prepared, low in sodium, and providing one-third of daily dietary requirements
Daily face-to-face visits by trained delivery drivers to check for signs of distress and contact a case worker if needed
Connection to a case worker for additional support and referrals
Meals for eligible caregivers for up to two weeks to help them focus on recovery and caregiving
Who Is Eligible
The PASST program serves adults 60 and older who live in Albany County and are recovering from hospitalization, rehabilitation or surgery, including procedures such as knee or hip replacement. Caregivers providing support may also receive meals to help during the recovery period.
Funding Options
The PASST program is funded by grants, individual donations and Eddy SeniorCare at St. Peter’s Health Partners. Services are provided based on available funding. Participation is free for those who qualify.
